THE WORK DOESN’T STOP AT DISCHARGE

Stay connected.
Learn what comes next.

Keep a familiar door open.

Life after treatment happens outside your walls. Regular text check-ins give people an easy way to stay in touch with the team they already know.

Collect more than anecdotes.

Build outcomes collection into your follow-up routine. Bring check-ins and structured surveys together, so your team has a record to review, not another spreadsheet to chase.

Make reporting part of the routine.

Keep responses, survey results, and cohort data organized for quality reviews and conversations with payers, referral partners, and accreditation teams.

ONE CONVERSATION

From the first check-in
to the next conversation.

Set the rhythm.

Personalize outreach tracks, messages, and timing for your program.

Hear what’s happening.

Alumni reply by text. Conversations stay connected to the person.

Follow through.

A rough reply texts and calls your contact. Your team decides the next step.

SAVINGS

What follow-up costs you now.

The figures are the hours your staff spend on follow-up calls, surveys by phone and outcome reports that Moira takes over.

How we calculate

A person on the default 12-month track gets about 19 contacts, and all your active cohorts are being followed at once, so a month’s contacts are your monthly discharges x 19 and its surveys are discharges x 4 (four per person a year).

Hours by hand: contacts x minutes per follow-up plus surveys x minutes per survey, over 60, plus the hours spent compiling outcomes.

Hours with Moira: the minutes your team still spends on replies, calls and surveys, over 60, plus the site’s monthly hours, from these assumptions:

  • 19 check-in texts per person.
  • 1 in 5 replies needs a person, at 5 minutes each.
  • 1 text in 20 becomes a call, at 15 minutes each.
  • 4 surveys per person, 2 in 5 still completed by phone, at 10 minutes each.
  • 5.5 hours a month per site for reading the Overview and reporting.
  • 1 site.

Hours saved is hours by hand less hours with Moira, never below zero; staff cost is hours saved x hourly cost; the percent is hours saved over hours by hand; full-time coordinators is hours saved over 173, a full-time month, said in words below one and rounded to the nearest whole number from one. Hours, dollars and the percent are rounded down.

U.S. median for substance abuse, behavioral disorder and mental health counselors: $59,350 a year, about $28.53 an hour, May 2025.

Research on continuing care finds that longer follow-up and more active outreach to people are associated with better results.

Built to support your reporting, not to replace your requirements.

The Joint Commission calls for standardized outcome measurement in applicable behavioral health services; organizations select appropriate instruments. Payer and accreditation requirements vary, and post-discharge surveys alone do not establish compliance.
